DEMENTIA CARE
When Alzheimer’s dementia strikes a loved one, it can be over-whelming to a caregiver. At Bridgewater Center for Rehabilitation and Nursing, we have designed a program to meet the special needs of our residents with Alzheimer’s disease or dementia.
- We offer a safe environment, balanced with state-of-the-art emergency and wander control systems for the protection of our residents and your peace of mind.
- Our caring and experienced staff is available 24 hours a day to provide for the needs of our Alzheimer and dementia residents, regardless of their unscheduled and varying health care needs.
- We provide assistance when it’s needed for tasks that have become too complicated, calm assurances when confusion and fear disorient, and an accepting environment where it’s perfectly fine to wander, enjoy a friendly chat or share a smile.
- We also know how important it is to support the family caregivers as a loved one’s personalities change and connections become more fragile.
Comprehensive Care Planning
Upon admission, the treatment team evaluates the resident’s condition and needs, and devises a Comprehensive Care Plan detailing each discipline’s interventions to meet the resident’s holistic needs. The goal is to promote the resident’s health and function at the maximum attainable levels. The team meets quarterly and upon any significant change in the residents’ status. The resident and /or their family are invited to attend these meetings to take part in the comprehensive care plan.
